Is a magazine-fed firearm loaded only when there's actually a round in the chamber, or if there's ammunition in the magazine with the chamber empty?

Legally, I don't think it matters if there's a round in the chamber or not.

Safety, it's *always* loaded unless it's in pieces on the table.

Practically, if there isn't a round chambered, it might as well not be loaded if you need to take a shot *right now*

Context is everything.

What I was taught is, if the magazine is in, the gun is loaded. Bullet chambered and safety off is ready to fire.
